We understand for young people with complex needs that transition can be a challenging time. Young people will move to adult services at 18, or up to 25 for some young people with education, health and care plans (EHCPs) with children’s occupational therapy and speech and language therapy. Adult services work differently to children’s services and our therapists will help guide and support young people to transition into adult services by:
- Encouraging self-management as much as able and/or ability to direct carers.
- Ensuring the young person has a EHCP / transition report to provide details of ongoing needs and support which they benefit from.
- Sharing our EHCP/ transition report and other supportive documents such as a therapy passport with the young person, their family, their GP and other professionals.
- Making sure the young person and their family understand their needs.
- Making sure the young person and their family are aware of how to manage potential future difficulties or where to access appropriate help should they arise.
- From the age of 14 years we will develop a therapy transition passport with the young person and their family, and update it as required, prior to discharge.
