Children's integrated asthma and wheeze service

  • HCT and West Hertfordshire Hospitals NHS Trust run an integrated asthma and wheeze nurse-led service that aims to improve outcomes and to support children, their families and local services with the long-term management of asthma/wheeze to prevent attacks. The nurses provide a collaborative approach as a community and hospital integrated team with children being seen in clinics according to clinical need.

How the service is provided

The specialist asthma nurses provide training and support to local services to support children, young people and their parents/carers to self manage their asthma or wheeze. The nurses will provide a collaborative approach as a community and hospital integrated team. Where appropriate, children can be seen in clinics according to clinical need and this will take place within 4 weeks following accepted referral to the service.

Who is eligible for the service

The service accepts patients aged two to 16-years-old who are registered with a West Hertfordshire GP and have presented on two or more occasions with exacerbation of pre-school wheeze/viral wheeze or asthma.

This is not an urgent care pathway; the child must have recovered from an attack and be clinically stable. For children who require a same day (or up to 48-hour) nursing review please follow the urgent care pathway.

Making a referral

Referrals are accepted Monday – Friday between 9am and 5pm by submitting a completed  asthma and wheeze referral form  and emailing it to hct.cypasthmawheeze@nhs.net.

The referral form can also be accessed via DXS by West Hertfordshire GPs.
