The Integrated Community Team (ICT) is a diverse group of professionals who come with a wide background of experience. The team consists of community nurses (may also be called district nurses), community matrons, physiotherapists, occupational therapists, specialist palliative care nurses, mental health nurses and health care and rehabilitation assistants. They provide care primarily to patients who are housebound and need to be seen in their own homes including residential and nursing homes. Examples of care the team deliver include:
- Wound care management
- Chronic disease management
- Palliative treatment and care
- Injections/eye drops
- Tissue viability e.g. treatment and prevention of pressure sores
- Leg ulcer management
- Teaching and support e.g. diabetes management
- Bladder and bowel management
- IV therapy
- PEG management
- Therapy assessments and treatments e.g. equipment provision, falls management, mobility assessments, treating orthopaedic condition, post-operative rehabilitation
The integrated community team service is from 8am to 10pm, every day of the year, including Bank Holidays.
The integrated community service is county wide, divided into locality teams covering geographic areas. In Herts Valleys, ICT is now known as community adult health services (CAHS).
Please note: A patient can self-refer within 3 months of being discharged from our service.

Referrals to this service are managed by the HCT led East and North Herts Referral Hub open from 8am to 8pm seven days a week. If you are a GP wanting to make a referral, please call 0300 123 7571 and select the option ‘For professionals’ to ensure your call is taken as soon as possible. Calls received between 8pm and 10pm will be diverted to locality teams.

How the service is provided
Patients are referred by a range of health and social care professionals for urgent intervention or routine care. Patients are assessed using a holistic approach to ensure that their individual health needs are identified, and to formulate a care plan or treatment programme. The team also considers the needs of the patient’s carer.
Who is eligible for the service
- Anyone over 18 years of age who is housebound and registered with a relevant GP
- Housebound patients with complex conditions
- Palliative and end-of-life care patients
- Patients with long term or complex conditions
What will happen at your appointment
- a telephone call to arrange a convenient visit time to plan your treatment and care
- advice, support, teaching and practical help with any of your health concerns
- advice on self-management of your health needs for the future
- visits by members of the Integrated Community Team to give specific course of treatment
- advice and support to you if you are acutely or chronically ill or have a disability
- advice regarding any equipment that will be of benefit to you and your carers
