The community stroke team offers specialist rehabilitation to patients who have suffered a stroke aged 18 years and over in East and North Hertfordshire, up to 6 months following the date of the stroke. Treatment is offered at home or in outpatient clinics, groups or virtually depending on need.
The pathways available are:
- Early Supported Discharge (ESD) home or clinic based rehabilitation for people who have suffered a mild to moderate stroke and have identified rehabilitation goals 0-6 weeks post stroke.
- Supported Discharge (SD) home or clinic based rehabilitation for people who have suffered a moderate to severe stroke with more complex needs requiring multidisciplinary input. Patients are discharged from acute hospitals, or inpatient rehabilitation settings.
- Disability Management (DM) support for patients who have suffered a severe stroke and require short term specialist support following discharge to support their optimal management in their care setting. These patients would not have rehabilitation goals.
- Six-month reviews for all stroke survivors in East and North Hertfordshire in partnership with the Stroke Association
The team accepts referrals from local hospitals, GPs and wider health professionals, tertiary centres and self referrals from patients and their families who have been seen by the service in the past year. Referrals should be sent to HCNT.EastandNorthHertsESD@nhs.net
