Hertfordshire Community NHS Trust (HCT) has created a “Digital Front Door” on the Trust website to streamline the referral processes into our Children’s and Adult Services. The new feature makes it quicker and easier for partners referring into HCT services to access the referral forms they need.

To use the Digital Front Door, simply visit Home | Hertfordshire Community NHS Trust and select ‘Make a referral’ in the ‘How can we help’ navigation bar or follow this link: HCT referral, adult, children | Hertfordshire Community NHS Trust to be taken directly to the digital front door homepage. 

It is hoped this new feature will enhance the user experience for referrers and support colleagues across the system to make timely referrals.

The Digital Front Door is part of an ongoing quality improvement project to improve access to care for patients, families, and carers in the local community HCT supports across Hertfordshire, west Essex, East Anglia, Bedfordshire, Luton and Milton Keynes.
