HCT at NHS Providers Annual Conference 2024

Our Hospital at Home service in east and north Hertfordshire, is one of the case studies featured in NHS Providers latest report on how NHS trusts are leading new approaches to ensure patients get the right care in the right place at the right time.

"Providers Deliver: shifting care upstream", shows how Hertfordshire Community NHS Trust and other trusts are innovating amidst a challenging environment, and adapting to deliver the three key shifts called for by the government, one of which is moving from hospital to community-based care. 
HCT has also developed a new project called Minus nine which builds on the work of our Hospital at Home service. Through this project the Trust is looking at how more proactive support can be provided for people through tailored interventions. Minus nine aims to identify what patients need nine days before intervention and to help prevent hospital admission where possible.  Minus 9 is one of two projects the Trust is presenting at NHS Providers Annual Conference this week.
Our Integrated Heart Failure Service in east and north Hertfordshire is also featured. Like  Hospital at Home, the Integrated Heart Failure service  focuses on a multi-disciplinary team working in partnership across the health and care system in east and north Herts, delivering services jointly to ensure care is patient-centred in our local communities.
