Integrated Community Teams (OTs)
Description of Service
The Integrated Care Team (ICT) provides care to our patients in their own homes and to try and prevent hospital admission as much as possible.
The ICT consists of Registered Nurses, Community Matrons, Healthcare Assistants, Assistant Practitioners, Occupational Therapists, Physiotherapists, Technical Instructors and Mental Health nurses.
Occupational Therapists aim to deliver services to patients in the ICT that may include:
- Provision of therapeutic rehabilitation
- Avoidance of inappropriate admissions to hospital
- Supporting early hospital discharge, where needed
- Supporting end of life care
Interventions may include:
- Graded practice of functional tasks - with a focus to return to independence
- Minor equipment provision to assist with daily independence
- Provision/review of moving and handling equipment
- Home environment reviews and identification of adaptations
- Education and advice for self-management of long term conditions
- Specialist assessments and joint working with additional professionals, as and when identified during treatment sessions – for example wheelchair and seating needs, or specialist items that will enhance quality of life.

We work with adults with a range of diagnoses including but not limited to Frailty, Falls, Neurological conditions, Cancer, Heart conditions, Dementia and Palliative conditions.
As Occupational Therapists, we focus on the occupational performance of the person, using comprehensive assessments to identify needs and create individualised treatment goals with our patients.
Areas covered
The ICTs cover the area of East and North Hertfordshire and are divided up into the following locality teams:
- North Herts
- Stevenage
- Welwyn & Hatfield
- Upper Lea Valley
- Lower Lea Valley
- Stort Valley and Villages
