AHP Practice Educators
Placement record form
All confirmed placements must be reported for Trust central records. This can be done by completing this form once a placement has been confirmed. If a placement is later cancelled then please report this to julie.demonte@nhs.net.
Practice Educator Training
We recommend renewing your practice educator training every 2 years, and no later than every 3 years. Your practice educator training must be provided by the Higher Education Institution of your student(s), unless HEIs have common assessment processes and paperwork.

Once you have attended practice educator training, you should record it on My Learning Zone using the following instructions:
- From the My Learning Zone homepage, click on ‘record of learning’ on the purple banner
- Click on the white tab ‘other evidence’
- Click the green button ‘add evidence’
- For the box ‘evidence name’ type (all lower case): ‘practice educator training [your profession*] [the training provider]’
*Type professions as:
dietetics
eye services
occupational therapy
physiotherapy
podiatry
speech and language therapy
You may add a description and attach documents for your own records. Please ensure you enable the date and add the date that the training was completed. - Click ‘add evidence’
